patients without dental insurance.
Just one annual payment!
After payment of an annual fee, members will receive the following:
1 Comprehensive Exam
Full Mouth Series XRAYS
2 Dental Cleanings
(in the absence of periodontal disease)
Panoramic XRAY
(if needed)
Emergency Visits: Problem Focused Exam and XRAYS (if necessary)
25% discount for cash payments
20% discount for credit card payments
No discounts apply if paying with Care-Credit
A treatment plan will be provided in writing for all recommended procedures displaying Discounted Rate

Coverage is in effect for one calendar year. Coverage begins when membership has been paid in full. Appointment times are limited, and it is the responsibility of the member to schedule appropriate visits.
